USEC Disaster Relief & Support Application

  • Please upload a photo for your ID, and a copy of your certification if you answered yes to any current certifications. If you cannot upload your photo and/or documents at this time, you may continue the process and email them to membership@usemergencycorps.org iOS does not support our upload system, so if you are on an iPad or iPhone, please use email.
  • If the form doesn't close with a message it was accepted, please double check that all required fields were completed and resubmit.
